Almost a year after the end of the 5th season you see Sam reappearing while saving Dean when he was poisoned by a djin . Sam reveals to Dean that he has been back for almost the whole year and that he has been hunting along side Samuel and their other family members that are related on their moms side. Sam appears to be colder in the episodes and has been lying to Dean. In the episode "The Third Man" Sam holds Dean back as Castiel tortures a boy to obtain information on the angel that holds the boy's soul. Dean asks Sam if he is okay and why he let Castiel to do such a thing as in the past Sam would have also fought against it.

In the episode "Live Free or TwiHard" you see Sam letting Dean to be turned into a vampire. After Samuel shows up and explains to Dean that there is a cure he confronts Sam telling him that he knew about the cure and questioned if Sam wanted Dean to be turned to find the Alpha Vampire. Sam lies to Samuel about it and seems disgusted that he would even ask such a thing. At the end of the episode Dean sees Sam only standing there and even smiling as he was being turned.

In the episode "You Can't Handle the Truth" Dean questions if Sam is actually Sam even wondering if he is Lucifer. Later in the episode Dean is able to get the truth because of the goddess Veritas' influence. When Sam is confronted he again lies to Dean, but Dean doesn't realize it. When Sam and Dean confront Veritas, she questions Dean and makes him tell the truth about how he feels about Sam. But when she questions Sam he is able to lie and she is angered and claims that he isn't human. Sam and Dean kill her and Sam is held at knife point by Dean. He explains that he knows that something is wrong with him and that he doesn't know, but explains that he doesn't feel anything and it makes him a better hunter. When he asks Dean for help Dean lowers the knife but still beats Sam unconscious. In the next episode it is shown that Sam does not have his soul anymore.
