A mysterious woman in an eye patch is out to kill angels. She seems to have some angel powers herself. Cas answers the prayer of an angel in need, only to find himself in her cross hairs. Sam and Dean need to unravel the truth in order to keep her from killing Cas for a past misdeed. |

Dean: OH, WELL, YOU CAN GO TO HELL. - Dean getting protective with Cas over how Ishim is talking to him.

Cas: LET'S DRINK, AND HOPE WE CAN FIND A BETTER WAY. - Cas in response to Sam and Dean asking him if he thinks he'll be able to go through with killing a nephilim when he has to.

- Will Lily ever come back to take revenge on Cas?
- What are the "cosmic consequences" of Cas killing Billie?
- How did Victorian-era Cas NOT know that the child he was sent to kill (May) was not a nephilim?
- Where did Lily's daughter May come from? She's human, but we never hear how she had her or who the father was.
- Just how highly Cas values the friendship of Dean and Sam; clearly valuing them over angels with whom he's had a very long history.
- Dean feels strongly enough about Cas that he can't stand to be left outside while Cas talks to Ishim.
- Angels hardly ever came to earth before they "fell".
- A human can learn how to use Enochian magic against angels, if they are willing to give up pieces of themselves every time they do it...and this magic can, apparently, make you very long-lived if not immortal.
- Though they are wary of Lily, even if the poses a threat to Cas, Sam and Dean are not willing to kill someone who does not want to directly harm them.
- "Angel radio" is still alive and well, in spite of the seemingly dwindling number of angels.
- Cas' previous vessel, before Jimmy, was a woman.
- Benjamin is playing the video game "Rampage"
- Dean refers to the baby Kelly Kline is carrying as "Rosemary's Baby" - a reference to a 1967 horror novel where a woman is unknowingly carrying Satan's child.
- Dean calls Lily "One-Eyed Willy" - a reference to the 1985 film "The Goonies"
